We Help You
At Willow Tree Counseling, we understand that healing and wellness often require support beyond
the counseling office. Our Targeted Case Management services help individuals and families
connect with resources and services that support their overall well-being, stability, and quality of
life.
This is a free service offered by Willow Tree Counseling to eligible clients and community
members through a referral process. Our goal is to ensure individuals have access to the
support and resources needed to overcome barriers and achieve their personal goals.
Our Case Managers work alongside individuals and families to identify needs, coordinate services, advocate for support, and navigate community systems that can often feel overwhelming. Case management services focus on helping individuals identify their needs, understand available options, and connect with appropriate community resources and supports.
Assistance may include connecting individuals with behavioral health services, community support programs, self-help and peer support resources, legal and advocacy services, employment and vocational programs, educational supports, resume-building resources, transportation connection assistance, food and basic needs programs, and other community-based services. Case managers may also help coordinate communication and collaboration among healthcare providers, schools, community agencies, and other support systems to ensure individuals have access to the resources they need.
